The single biggest thing that moves a dental cleaning quote anywhere in India is not the clinic's postcode — it is whether you need a routine clean above and just below the gumline, or deep cleaning inside pockets that have already formed around the roots. Those are different procedures with different requirements.

That distinction explains most of the confusion people run into when they ring three clinics and get three answers. Below is what each of the real variables does to the number, so you can tell whether two quotes are actually comparable before you decide which is better value.

Why do two quotes for "a cleaning" differ so much?

Usually because one of them is a routine appointment and the other is not.

Scaling and polishing is the routine version: hardened deposits removed from the tooth surfaces and from along and just below the gumline, then polishing to smooth the surfaces so plaque is slower to re-adhere. For most people it is one appointment, needs no anaesthesia, and has no downtime.

Advanced gum treatment is what is required once pockets have formed around the teeth and the supporting bone has begun to be lost. It starts with full periodontal charting — pocket depths measured around every tooth, bleeding points recorded, mobility assessed and X-rays taken to see the bone level. The cleaning itself is deep scaling and root planing under local anaesthesia, usually a quadrant at a time, and it is followed by a reassessment to see which sites responded.

Those two are quoted differently because they are different amounts of clinical time, different equipment, and different levels of risk. A quote that looks unusually low may simply be for the first thing when you need the second, and the honest way to find out is to have the pockets measured rather than to compare headline figures.

How much does the number of visits move it?

A lot, and this is where the two procedures separate most clearly. Routine scaling and polishing is usually a single appointment. Advanced gum treatment runs to several visits, often split by quadrant, plus maintenance appointments afterwards.

So the useful question is never "how much is a cleaning" but "how many appointments does my mouth need, and what happens at each". A single figure quoted without that context is not really a price.

Does anaesthesia change what I pay?

It can, because it reflects what is being done rather than being an optional extra. Routine scaling does not usually need anaesthesia, though numbing is available if you are sensitive — for most people the experience is discomfort rather than pain, mainly where the gums are already inflamed. Deep cleaning and any surgical step in advanced gum treatment are done under local anaesthesia as standard.

If a quote includes anaesthesia, that is a signal about the depth of the procedure, not about the clinic padding the bill.

What is actually included in the fee — and what is not?

This is where most like-for-like comparisons quietly fall apart. A few things worth pinning down before you accept any figure:

The examination. A cleaning appointment is also an examination — teeth and gums are checked for plaque, tartar, bleeding and any signs of gum disease or decay. That is part of what you are paying for, and it is the part that catches small cavities, a cracked filling or early gum recession while they are still cheap to deal with.

Diagnostics. X-rays are taken at intervals set by risk, to see between and beneath teeth. In advanced gum treatment, charting and X-rays are the first step rather than an add-on. Whether imaging sits inside or outside the quoted fee varies, and it is a fair thing to ask.

Anything the examination finds. The cleaning fee covers the cleaning. If a filling, a cracked restoration or gum disease is found, treating it is a separate matter with its own cost. That is not a hidden charge; it is the appointment doing its job.

Home care guidance. Being shown the three specific places you personally keep missing is worth more than generic advice, and it is part of the appointment rather than a separate service.

The consultation itself. Ours is ₹500, waived off against treatment.

Does it matter who performs it?

Routine scaling and polishing is performed by a dentist or a dental hygienist. Advanced gum treatment is performed by a dentist, always — as is any laser work involved in it. Practices staffed differently will price differently, and that is a legitimate difference rather than a trick, but it is another reason two figures may not be describing the same appointment.

Does the technique change the price?

An ultrasonic scaler does most of the work in a routine clean, using high-frequency vibration and a water spray to break tartar away from the tooth rather than scraping it off, with hand instruments used only for what the tip cannot reach. That is the standard approach and it is what the standard fee buys.

Where pockets persist after deep cleaning, treatment moves to the specific sites that need it — laser therapy, minimally invasive surgical access, or regenerative and grafting procedures. Whether a laser costs more than the conventional alternative depends on the procedure, and the question that actually matters is which technique suits your case clinically. A dentist should set out both options and what each involves at the consultation, rather than presenting the more expensive one as automatically better suited.

Does the city change it?

Fees do vary between cities and between types of practice, and we do not publish a national range because we do not have reliable data for one — quoting a figure we cannot stand behind would be worse than saying so.

What is comparable across any two clinics is scope: how many appointments, whether anaesthesia is involved, whether imaging and charting are included, who performs it, and what happens if the examination finds something. Compare those and the price difference usually explains itself.

What does it cost over time rather than per appointment?

This is the number that matters more, and it is the one nobody quotes.

Most people need scaling and polishing every 6 to 12 months, with a shorter interval if you form tartar quickly, smoke, or have a history of gum disease. After advanced gum treatment the maintenance interval is typically every three to four months, and it is not optional — gum disease recurs when maintenance lapses, and treatment that is not followed up largely returns to where it started over a few years.

Preventive dentistry makes the arithmetic explicit: two examinations a year cost far less than the fillings, root canals and crowns they prevent, because decay and gum disease cause no symptoms in their early stages and by the time a tooth hurts the simple window has usually closed. Fluoride application and fissure sealants, where they are indicated, work on the same principle — small, planned spending in place of large, unplanned spending. Prevention lowers the risk substantially; it cannot rule decay or gum disease out entirely, since diet, medication, dry mouth and genetics all continue to matter.

Put crudely: the cheapest scaling appointment is the one you have on schedule, because the expensive version is the one you need after five years of not having any.

What to ask before you compare two quotes

How many appointments is this figure for? Is it a routine clean or deep cleaning below the gumline? Have my pockets been measured? Are X-rays and the examination included? Who will perform it? And what is the recall interval you would set for me specifically, rather than as a default?

If a clinic can answer those six, you can compare their number to anyone else's. If it cannot, the number is not the problem.
