The four things usually being checked
The core issue in every case is the same: an implant only works if bone and gum heal properly around it. Everything on this list is there because it affects healing.
- Uncontrolled diabetes. The word doing the work is "uncontrolled" — this is about how well blood sugar is managed, not about having diabetes.
- Active gum disease. Placing an implant into an actively inflamed mouth puts it into the same conditions that damaged the natural teeth.
- Heavy smoking, which affects blood supply and healing at the site.
- Insufficient jawbone, which is about volume of bone available to hold the implant securely.
Most of these are sequencing, not refusal
This is the part usually missed. Active gum disease is treated first, and then implants are reconsidered. Blood sugar control is something to work on with your physician. These change when rather than whether.
Reading a list like this and quietly concluding you are not eligible is the most common wrong outcome, and it is why the assessment exists.
Not enough bone is often solvable
Bone loss where a tooth has been missing for a long time is common, and it does not usually end the conversation. A bone graft builds up the site so an implant can be placed. Whether you need one is decided from imaging at the planning appointment, not from how the gap looks.

Assessment and imaging come first, then a treatment plan. The scan is what answers the bone question; your medical history and a gum examination answer the rest. If an implant genuinely is not the right treatment, that is when you should be told — and alternatives such as a bridge or a denture discussed rather than left unsaid.
What to bring up
Tell the dentist about any medication you take, any diagnosed condition, whether you smoke and how much, and how long the tooth has been missing. That last one predicts the bone question better than anything else you can report.
