Is wisdom tooth removal painful?
The extraction itself is done under anaesthesia, so you should not feel pain while it happens — pressure and movement, but not pain. Afterwards is the part people mean: soreness and swelling for several days, peaking around day two or three, managed with prescribed pain relief and aftercare. Pain that increases after the third day is the one pattern that is not normal.
During the procedure
You should not feel pain during the extraction. The area is fully anaesthetised and what you notice is pressure, movement and the dentist working. For most people the appointment is less unpleasant than the anticipation of it.
Afterwards, which is what people mean
Soreness and swelling for several days is normal after a surgical extraction. The pattern is predictable and worth knowing so it does not alarm you:
- Swelling peaks on day two or three, then settles. It usually looks worse before it looks better. Cold compresses in the first day help.
- Limited jaw opening for a few days.
- Some oozing on the first day, controlled by biting on gauze.
- Bruising, sometimes.
Prescribed pain relief and the aftercare instructions are what carry you through it.
Dry socket — the reason for the horror stories
If the blood clot in the socket is lost, the site becomes acutely painful a few days after the extraction. This is the specific thing behind most accounts of wisdom teeth being unbearable, and it is treatable.
More usefully, the risk is reduced by three things you control in the first days: do not smoke, do not rinse vigorously, and do not drink through a straw. Avoid very hot food and alcohol as well, and stay on soft, cool foods away from the extraction side.
The rule that separates normal from not
Pain that eases day by day is healing. Pain that increases after the third day is not, and neither is bleeding that does not settle, fever, spreading swelling, a bad taste, or difficulty swallowing. Call the clinic — none of these are things to wait out at home.
What makes it easier
Timing helps where there is a choice: late teens to early twenties is often easier, because the roots are less fully formed and the bone is less dense. And a tooth that is straightforward on the X-ray generally has a shorter, milder recovery than one deeply impacted.
