What deep cleaning is, and why anaesthesia is used

Routine scaling cleans the tooth surfaces at and just below the gumline. Deep cleaning goes further — into the pockets that have formed around teeth where gum disease has already affected the supporting bone.

Because it works below the gumline on root surfaces, it is done under local anaesthesia. The appointment itself should not be painful. It may also involve laser or surgical access depending on the case.

The days afterwards

• Tender gums and minor bleeding for a few days. Persistent or heavy bleeding should be reported.
• Cold sensitivity on the cleaned root surfaces, which is the effect that lasts longest — commonly several weeks. A desensitising toothpaste helps and it generally settles.

Deep cleaning usually means a few days of tenderness overall. Surgical or regenerative procedures take longer — often a couple of weeks for the gums to settle, and months for the underlying healing.

Two things that surprise people

Gums recede as the inflammation resolves. Swollen gums shrink back once treated, so teeth can look longer and gaps more visible afterwards. This is the disease becoming apparent rather than the treatment causing it — but it is genuinely worth expecting, because it can look like the treatment made things worse.

Teeth can feel slightly looser at first. Some increased mobility immediately after treatment is normal and usually improves as the tissues firm up. Where bone loss is advanced, some mobility may remain.

What makes the outcome better or worse

Smoking and diabetes both substantially reduce how well gums respond and how well the results hold. This is discussed honestly at assessment because it changes what is realistic, not to assign blame.

The part that decides whether it was worth it

Gum disease is controlled rather than cured. Maintenance visits — usually every three to four months — are what keep it from progressing again, and treatment that is not followed up largely returns to baseline over a few years.
