Do clear aligners hurt?

It is the feeling of teeth under steady pressure rather than sharp pain, and it settles within a few days. Most people find the first few trays the most noticeable and later ones barely register, because each tray moves the teeth only a small amount.

What it actually feels like

Pressure, not sharp pain. Teeth are being moved through bone, slowly, and that registers as tenderness — most noticeably when you bite down in the first day or two after a new tray goes in.

It settles on its own within a few days, and then the tray is doing its work quietly until the next one.

The rhythm of it

Aligner treatment has a predictable cycle. Each new tray is slightly different from the last, so there is a short adjustment period, then comfort, then the next tray. People usually report that the first few are the most noticeable and that later ones are barely worth mentioning.

Fixed braces follow the same pattern around fitting and each adjustment appointment rather than around tray changes.

The other discomfort, with braces specifically

Brackets and wires can rub the inside of the cheek or lip early on, before the mouth adapts. Orthodontic wax is what that is for. Aligners are smooth trays and do not have this problem, which is one of the genuine comfort advantages they hold.

When soreness means something else

Ongoing pain rather than settling tenderness is worth a call, as is a tray with a sharp edge, a bracket that has come loose, or a wire poking into the cheek. None of those should be endured until the next appointment.

Sensitivity that arrives with decay is a different problem again — fixed braces trap food and plaque, and without careful brushing and flossing there is a real risk of decay and white marks on the enamel. Pain that feels like a cavity should be looked at as one.

Some discomfort is unavoidable, because the discomfort is the treatment working. What should not happen is pain that does not settle, and what makes the whole course easier is the cleaning routine, which does more for comfort than anything else you can do at home.
