Masseter Botox (Jaw Slimming) Aftercare: The First Week, Day by Day

You can leave straight after the appointment. Avoid rubbing or massaging the jaw for the first twenty-four hours, and skip strenuous exercise for the remainder of that day. Chewing can feel slightly tired for the first few days as the muscle relaxes. Nothing visible happens in week one — softening only begins from around two weeks.

That last sentence is the one that catches people out, so it is worth saying before the day-by-day: the first week of Masseter Botox (Jaw Slimming) is not where you look for results. It is where you look after the injection points and get on with your life.

What should you do on the day itself?

Very little, deliberately. You are free to leave as soon as the injections are done — most patients go back to work the same day, since downtime here is minimal.

Two rules apply from the moment you walk out. Keep your hands off the jaw: no rubbing, no massaging, no pressing to check whether anything has happened yet, for a full twenty-four hours. And drop the strenuous exercise for the rest of the day; the gym can wait until tomorrow.

Eating is normal on day one for most people. If anything feels different, it tends to be a faint tiredness in the jaw rather than difficulty.

What does day one look like?

Ordinary, with a slightly tired jaw. This is the point at which chewing fatigue usually announces itself — the commonest effect of this treatment, and one that behaves exactly as the name suggests. Tough or chewy food takes more effort than it did. Soft food does not.

The reason is straightforward. The masseter is doing less, and the muscles around it are taking up more of the work while everything rebalances. That hand-over is the fatigue. It settles within days to a couple of weeks.

The injection points may be a little tender, and a bruise may be visible. Bruising is occasional rather than expected, and is likelier if you take blood-thinning medication or fish oil. It generally clears within a few days.

Days two and three — what is normal?

The chewing effort usually peaks somewhere in here and then starts easing. This is a good stretch for softer meals: dal and rice, curd rice, idli, eggs, anything that does not need to be worked at. Nobody has prescribed a diet — it is simply the path of least resistance for two or three days.

A short-lived headache is occasionally reported in the first days after botulinum toxin treatment and passes on its own. If you have had toxin elsewhere on the face before, this may already be familiar.

You will not see a slimmer jaw in the mirror on day three. Nobody does. The muscle has to physically reduce once it stops overworking, and that is a process measured in weeks.

Days four to seven — what changes?

Mostly, things stop being noticeable. Chewing normalises for most people towards the end of the first week as neighbouring muscles settle into their share. Any bruising should have faded. Normal exercise resumed after the first day is entirely fine.

If you were treated partly for clenching, this is around when the first hints sometimes arrive — a jaw that is less tight on waking. The clinic's own timeline puts grinding relief at within a couple of weeks, so late in week one is early rather than late, and its absence at day seven means nothing has gone wrong.

What should you avoid during the first week?

Three things, and only for as long as stated:

  • Rubbing or massaging the jaw — for the first twenty-four hours.
  • Strenuous exercise — for the remainder of treatment day.
  • Testing it constantly. Clenching hard in front of a mirror twice a day tells you nothing at this stage and makes an unremarkable week feel like a failed one.

Beyond that there is no long list. This treatment does not come with a week of restrictions, which is part of why people choose it.

When should you contact the clinic?

Most first weeks are uneventful. A few things are worth a phone call rather than waiting to see — and the point of ringing is precisely that you should not be assessing them yourself:

  • Chewing fatigue that is getting worse rather than easing, or that is still with you beyond a couple of weeks.
  • Bruising or tenderness at an injection point that has not settled within a few days.
  • Anything at an injection point heading the wrong way — increasing redness, swelling, warmth or pain.
  • Any change in your smile, or a visible difference between the two sides.
  • A headache that outlasts the first few days.

None of those is a diagnosis, and this article is not the place to work out what any of them means. The clinic that examined and injected you is. Call them, describe it plainly, and let them decide whether you need to be seen.

Separately, and it matters: difficulty swallowing, speaking or breathing is not an expected effect of this treatment at any point. If it occurs, seek medical care straight away and tell the clinic — do not wait for a scheduled review or look for reassurance online first.

What happens after the first week?

The interesting part. From around two weeks the muscle is measurably relaxing, which is usually when patients treated for clenching notice easier mornings. Visible slimming builds over four to six weeks. A review is set at around six weeks to check symmetry and decide whether any small top-up is needed — and because the change is gradual, that review is a far better judge of the result than your own mirror at day ten.

From there, the effect typically lasts four to six months before the muscle gradually returns to its previous activity, at which point the question becomes whether to repeat.
